How to read a result
- Enter the NPI if you have it. A hit on that number is a confirmed row for that number.
- If you only have a name, enter the last name and the first name.
- Read the list. LEIE and SAM are labeled on the row.
- If the row has no NPI, the badge stays Unconfirmed. It is not stored as excluded.

What is the LEIE?
The LEIE is the HHS Office of Inspector General’s List of Excluded Individuals and Entities. People and organizations on it are excluded from federal health care programs such as Medicare and Medicaid. OIG replaces the public file monthly. This search reads the September 2026 file, dated 10 September 2026, and the SAM public extract dated 22 September 2026. A result is the list row. How does someone get on the OIG exclusion list?
OIG excludes under sections of the Social Security Act. Common grounds include certain criminal convictions, patient abuse, health care fraud, and license revocation or surrender. The row names the exclusion type and the date. This page does not summarize the underlying case or the evidence. Read the type on the row, then open the OIG record if you need the program details. A matching name, without the same NPI, is not enough to treat the person as excluded.
How often should I check the OIG exclusion list?
Check before someone starts, and check again every month while they are on the roster. Federal guidance treats the monthly check as the ordinary cadence, because OIG replaces the file on that schedule. SAM’s public extract is daily. Monitoring on this site reruns a roster of up to 50 people against the loaded files and emails a change when a compared field moves. A name-only hit is not emailed as a confirmed exclusion.
