Each lookup names the file it read. Match a date here when you need to see how fresh the check was.
Plainroster is not the source agency. This is not primary-source verification. A credentialer may still open the board’s own site.

How a row is matched
We match on NPI when both sides have one, or on the state plus the exact license number. A name alone stays unconfirmed, so you can compare it before anyone treats the person as excluded.
Status labels
Every status has a word and a mark. Color is never the only signal.
Active means the file says active. Expired means the file says so, or the date has passed. Suspended means the file uses that word. Excluded means a confirmed match to LEIE or SAM. Unconfirmed means a name match only. Unknown means the file has no status we can read. California “Delinquent” is printed with that word: the license was not renewed by the expiration date.
