Each lookup names the file it read. Plainroster is not the source agency. This is not primary-source verification. A credentialer may still open the board’s own site.

Matching
A provider record joins on NPI when both sides have one, or on state plus the exact license number. A name alone stays unconfirmed. It is not stored as excluded, and it is not emailed as a confirmed match.
